Resumo: Proliferative leukoplakia (PL) is characterized by high malignant transformation rates and frequent clinicopathologic discordance. We report two cases in which clinical findings raised strong concern for malignancy despite inconclusive histopathological results.
A 73-year-old woman presented with multifocal, exophytic white lesions involving the buccal mucosa, alveolar ridge, floor of mouth, and ventral tongue. Initial and repeat biopsies showed verrucous changes with dysplasia or hyperkeratosis without malignancy. Given persistently concerning clinical features, she underwent high-frequency electroporation with bleomycin, during which intraoperative biopsies revealed moderately differentiated oral squamous cell carcinoma (OSCC).
A 77-year-old woman with oral lichen planus developed a large verrucous buccal lesion. Multiple biopsies over 3 months showed mild to moderate dysplasia. Computed tomography revealed a mass with mandibular erosion and suspicious cervical lymph nodes. Intraoperative sampling confirmed well-differentiated OSCC.
These findings highlight the limitations of incisional biopsy in PL and support management guided by clinical suspicion when discordance persists.
