Resumo: Granulomatosis with polyangiitis (GPA, formerly Wegener's granulomatosis) is a rare ANCA-associated vasculitis with heterogeneous clinical presentation, often leading to delayed diagnosis. We report three cases to describe oral manifestations that contributed to establishing the diagnosis. A 69-year-old woman presented with progressive gingival enlargement consistent with "strawberry gingivitis," unresponsive to antibiotics and associated with systemic symptoms. Gingival biopsy showed chronic inflammation, microabscess foci, and pseudoepitheliomatous hyperplasia; imaging revealed multiple pulmonary nodules. Following histopathologic review demonstrated necrotizing vasculitis with multinucleated giant cells, and PR3-ANCA positivity with lung biopsy confirmed GPA. A 46-year-old woman with longstanding multisystem involvement, including recurrent oral ulcers and mandibular osteitis, experienced delayed diagnosis due to overlapping features. In a third case, findings included PR3-ANCA positivity with involvement of the nasal septum, paranasal sinuses, and hard palate perforation. These cases illustrate the spectrum of oral manifestations in GPA and demonstrate how oral findings may contribute to establishing the diagnosis.
