Título: ATYPICAL PRESENTATION OF SQUAMOUS CELL CARCINOMA IN A PATIENT PREVIOUSLY DIAGNOSED WITH ORAL LICHEN PLANUS

Resumo: A 78-year-old female patient presented in an Oral Medicine service with pain and burning sensation in the mouth. Physical examination revealed multiple lesions with striated and erosive areas involving the bilateral buccal mucosa, tongue, and gingiva. An incisional biopsy on the buccal mucosa was performed. Histopathological diagnosis was consistent with lichen planus (LP) and the patient was treated with topical corticosteroids. Four years later, the patient returned with a painful mass involving the right mandible and bilateral submandibular lymphadenopathies. Intraoral examination showed an expansive, bleeding lesion involving the right mandibular alveolar ridge, and multiple white plaques on the tongue. Panoramic radiograph demonstrated extensive bone destruction with a pathological fracture. Histopathological analysis established the diagnosis of undifferentiated squamous cell carcinoma. This case raises the possibility that the previous diagnosis of OLP may have corresponded to lichenoid proliferative verrucous leukoplakia.
