Resumo: Two cases of odontogenic keratocyst in female patients in the second decade of life are presented, both affecting the posterior mandible and associated with impacted third molars. In the first case, the lesion was incidentally diagnosed during orthodontic evaluation, revealing a multilocular radiolucent lesion involving the mandibular body and ramus, and was treated with enucleation associated with tooth extraction. In the second case, the patient presented with pain and paresthesia, with an extensive lesion in close relationship with the mandibular canal, initial management involved decompression due to lesion size and risk of pathological fracture, followed by enucleation after six months.The investigation included panoramic radiography, cone-beam computed tomography, and histopathological confirmation. Both cases showed recurrence after six months, requiring further surgical intervention. These cases highlight different therapeutic strategies and emphasize the need for individualized treatment planning and strict clinical and radiographic follow-up.
