Resumo: Orthokeratinized odontogenic cyst (OOC) is an uncommon odontogenic cyst that was classified as a distinct entity in 2005. A few reports have shown tha OOC can undergo malignant transformation. In the present case we report a case of OOC that presented marked epithelial dysplasia. The patient was 62-year-old white male who presented a 3 cm radiolucent lesion in the anterior maxilla, with a 6-month evolution. . Computed tomography was performed and showed that the lesion was not associated with a tooth. With a provisional diagnosis of nasopalatine duct cyst, an excisional biopsy was conducted. Histopathological examination revealed a single fragment (35 × 28 × 27 mm), with rubbery consistency and irregular surface. The cystic capsule was lined by orthokeratinized epithelium, with focal areas showing blunt epithelial projections, acanthosis, and pleomorphic cells. The final diagnosis was orthokeratinized odontogenic cyst with epithelial dysplasia. The patient is under follow-up
