Resumo: This report describes the case of a 53-year-old female patient, ASA II, with hypertension and a diagnosis of Gorlin-Goltz syndrome, under prolonged clinical follow-up due to the occurrence of recurrent odontogenic keratocysts. Initially, the patient presented with a lesion in the mandibular region, which subsequently progressed to maxillary involvement. Radiographic and tomographic examinations revealed a small to medium-sized, well-defined, unilocular radiolucent image in the posterior region of the maxilla, measuring approximately 14.6 mm in the coronal section and 23.5 mm in the sagittal section, without significant bone expansion. Excisional biopsy was performed as a diagnostic method, with anatomopathological confirmation of a parakeratinized odontogenic keratocyst exhibiting aggressive behavior. Treatment consisted of enucleation combined with curettage. The patients history of relapses over the years highlights the need for continuous clinical and radiographic follow-up, due to the high potential for recurrence in syndromic patients.
