Título: FROM MINOR ORAL TRAUMA TO FACIAL NECROTIZING FASCIITIS IN A NEUTROPENIC PATIENT

Resumo: Necrotizing fasciitis is a fulminant, life-threatening infection characterized by rapid soft tissue destruction and high mortality, especially in neutropenic patients. This study aims to highlight the risk of severe necrotizing infection arising from minor oral trauma in immunosuppressed individuals. A 19-year-old male with refractory B-cell acute lymphoblastic leukemia developed facial pain and edema after gingival trauma during toothbrushing. The condition rapidly evolved to extensive necrosis of the right hemiface, with loss of buccal mucosa, cheek soft tissues, and maxillary bone exposure. Computed tomography demonstrated a collection extending from the retropharynx to the mediastinum. Due to thrombocytopenia and airway compromise, prophylactic tracheostomy was required. The patient progressed with decreased consciousness and Klebsiella pneumoniae carbapenemase bacteremia, requiring ICU admission. Management included broad-spectrum antibiotics, serial debridement, hyperbaric oxygen therapy, and reconstructive surgery. Biopsy confirmed necrotizing fasciitis. Even minimal oral trauma may trigger catastrophic infections in neutropenic patients, demanding immediate recognition and multidisciplinary management.
