Resumo: Peripheral manifestation of the calcifying odontogenic cyst (COC) is rare, typically presenting as a slow-growing, asymptomatic swelling. Due to its soft tissue location, radiographic evaluation may be limited, hindering the identification of internal structures and calcifications. A 61-year-old male presented with a 5cm, well-circumscribed, normochromic, and fluctuant swelling in the right upper gingiva, persisting for 10 months. Cone Beam Computed Tomography showed an extraosseous isodense lesion with adjacent cortical resorption. High-frequency ultrasonography (extraoral and intraoral linear probe) revealed hyperechoic areas suggestive of calcifications, significantly contributing to the diagnostic hypothesis of peripheral COC. Excisional biopsy followed by histopathological analysis confirmed the diagnosis. Treatment consisted of surgical intervention, and the patient showed favorable recovery with no recurrence during follow-up. This case highlights ultrasonography as a valuable complementary tool, providing greater diagnostic precision by detecting internal structures not clearly visualized by conventional imaging, thus assisting in the preoperative characterization of peripheral odontogenic lesions.
