Resumo: Objective: To analyze the clinical and histopathological characteristics, as well as prognostic factors of gingival metastases (GM). Study Design: Case reports and retrospective studies of GM with histopathological confirmation were searched in PubMed, Embase, Scopus, Web of Science, and gray literature. Results: A total of 169 studies (253 patients) were included. There was a predominance of male patients (n = 157; 62.1%), with a mean age of 57.5 years. Lesions most frequently occurred in the posterior mandibular gingiva (n = 64; 25.3%). Regarding the primary tumor, in males the most frequent sites were the lung (34.4%) and kidneys (12.1%), while in females the most common were the breast (24%) and lung (19.8%). The main histopathological subtypes were carcinoma (n = 129; 51%) and adenocarcinoma (n = 80; 31.6%). Kaplan-Meier curves estimated 4-year overall survival (OS) and disease-specific survival (DSS) rates of 6.1% and 19.9%, respectively. The log-rank test showed higher DSS in female patients (p = 0.021). Univariate Cox regression identified sex as a prognostic factor (HR = 0.61; p = 0.0214), with lower survival in males. Conclusion: GM are rare but reflect aggressive biological behavior, being associated with poor prognosis and low overall survival rates.
