Título: BULLOUS PEMPHIGOID: A MULTIDISCIPLINARY CASE REPORT

Resumo: A 55-year-old male presented multiple ulcerations affecting palate, buccal mucosa, and ventral tongue, severe pain, dysphagia, and negative Nikolsky sign. He had been treated for posterior laryngitis five months prior due to a bleeding vocal cord lesion and enanthematous pangastritis. The condition progressed to oral lesions followed by bullae on the scalp, limbs, and trunk. Oral biopsy revealed ulcerated chronic inflammatory process (CIP); negative PAS and Grocott stains. Endoscopy showed edematous epiglottis, polypoid vocal cord lesions, mild distal erosive esophagitis, and moderate flat erosive gastritis. Duodenal and gastric biopsies indicated mild CIP without atypia, preserved villi, and moderate inactive chronic gastritis. Patch testing was positive for casein, egg white, Peru's balsam, fragrances, nuts, soy, paraphenylenediamine, and triethanolamine. HIV, syphilis, ANA, and ANCA were negative. Skin biopsy confirmed bullous pemphigoid. Treatment with prednisone for 10 days yielded a poor response; subsequent monthly 1mL Permese injections and dietary led to clinical improvement.
