Resumo: This case report describes a 59-year-old man presenting with a painful tongue lesion for three months after accidental trauma. Clinical examination revealed a chronic granulomatous ulcer with a mulberry-like appearance with raised and indurated borders. There was also a lesions on the soft palate, multiple punctate hemorrhagic areas were observed on the surface. An incisional biopsy of the tongue lesion and exfoliative cytology of the palate were performed. Histopathology showed chronic granulomatous inflammation containing multibudding yeasts, consistent with paracoccidioidomycosis. Medical evaluation confirmed pulmonary involvement. Systemic antifungal therapy was initiated, with favorable clinical outcomes. This case highlights the importance of recognizing oral manifestations to enable diagnosis and appropriate management of systemic fungal infections.
