Resumo: Synovial sarcoma (SS) is a rare malignant soft tissue neoplasm characterized by aggressive behavior and an uncommon occurrence in the oral cavity. A 45-year-old woman presented for evaluation of a painful lesion located in the posterior region of the left mandibular alveolar ridge, with a one-month history of progression. Clinical examination revealed an erythematous, bleeding tumor with irregular contours and areas of necrosis. Computed tomography showed an ill-defined lesion with cortical bone perforation. An incisional biopsy was performed, which revealed a monophasic tumor composed of atypical spindle cells, relatively uniform in appearance, arranged in short multidirectional fascicles with interspersed tortuous vessels. Immunohistochemical analysis showed positivity for CD56, AE1/AE3, TLE1/SS18-SSX, BCL2, vimentin, and high Ki67 expression, consistent with the diagnosis of synovial sarcoma. The patient was referred for surgical excision of the lesion and remains under follow-up. (Support: FAPEMIG/CAPES/CNPq).
