Título: SALIVARY DUCT CARCINOMA ARISING FROM SCLEROSING POLYCYSTIC ADENOMA OF PAROTID GLAND

Resumo: Sclerosing polycystic adenoma (SPA), a rare benign salivary gland neoplasm initially considered a non-neoplastic lesion resembling fibrocystic breast disease, recently has been reported with malignant transformation. Here we present the case of a 56-year-old man who was admitted to the hospital with a slowly growing mass in the left parotid region. A superficial parotidectomy was performed and histopathological examination revealed a cystic epithelial proliferation within a sclerotic collagenous stroma, with well-circumscribed ductal structures showing sebaceous metaplasia, apocrine differentiation and focal lymphoid aggregates. Additionally, invasive neoplastic ductal proliferation, and perineural invasion were observed. Immunohistochemical staining showed positivity for SOX10, CK7, S100, and p63 in tumor cells. The final diagnosis was SPA with malignant transformation into salivary duct carcinoma; thus, complete resection was performed; clinical follow-up over 2 years showed no evidence of recurrence. These findings highlight the importance of recognizing malignant features in this unique entity.
