Resumo: Actinic cheilitis (AC) is a potential malignant disorder where biopsy is important to determine the degree of epithelial dysplasia and guide treatment. A 65-year-old female with a heterogeneous lower semimucosa, showed hypochromic and brown macules areas that are compatible with AC. She reported a lip augmentation with polymethylmethacrylate (PMMA) and hyaluronic acid filler in 1998. An ultrasonography scan was requested to map a safer area and avoid the tissue containing PMMA. In conjunction with physical and wide-field fluorescence (MMOptics, São Paulo, Brazil) exams, the area was selected for incisional biopsy. Mild epithelial dysplasia was diagnosed and the patient remains under clinical follow-up. This report highlights the importance of using different tools to assist the better area for biopsy in patients with PMMA, emphasizing the challenge of selecting a safe and representative area, as well as the importance of a multidisciplinary approach.
