Título: LATE DIAGNOSIS OF MANDIBULAR ANGIOSARCOMA INITIALLY PRESENTING AS A PERSISTENT POST-EXTRACTION BLOOD CLOT: A CASE REPORT

Resumo: A 77-year-old man developed a persistent, non-healing hemorrhagic clot following mandibular tooth extractions in July 2024. One year later, referral to a Stomatology unit, the clinical examination revealed exposed necrotic bone at the mandibular symphysis, pronounced anterior dental mobility, mental nerve paresthesia, and halitosis. His medical history was notable only for minimal tobacco exposure during childhood, with no prior use of antiresorptive, antiangiogenic, or head and neck radiotherapy. Laboratory and serological evaluations were unremarkable. Computed tomography demonstrated an extensive infiltrate, hypodense lesion in the anterior mandible, exhibiting marked cortical destruction and bone sequestration. The initial differential diagnosis included osteonecrosis or malignant bone neoplasm. An incisional biopsy was performed, and histopathological analysis revealed a malignant spindle cell and epithelioid neoplasm. Immunohistochemical profile showed AE1/AE2+/- and p63+, CD31+, CD34+, ERG+, FLI-1+, Factor VIII+, confirming the diagnosis of epithelioid angiosarcoma. The patient received palliative radiotherapy but died two months post-diagnosis.
