Título: LARGE AMELOBLASTIC FIBROMA IN A CHILD: A FAVORABLE OUTCOME WITH CONSERVATIVE MANAGEMENT

Resumo: An 11-year-old girl presented with asymptomatic swelling of the left mandible of unknown duration. Clinical examination revealed a normochromic swelling in the posterior lower vestibule. Imaging demonstrated a 3.5 cm, well-defined, multilocular radiolucency associated with an unerupted developing second premolar, involving the mandibular body and causing thinning of the basal cortical bone. Incisional biopsy was performed and microscopic analysis showed strands and islands of odontogenic epithelium embedded in a cellular ectomesenchymal stroma resembling dental papilla, without evidence of mineralized tissue formation. The final diagnosis was ameloblastic fibroma. The patient was treated with curettage and extraction of the involved tooth. At 6-month follow-up, the patient shows progressive bone regeneration, satisfactory healing, and no evidence of recurrence. This case highlights that ameloblastic fibroma may reach considerable size while still allowing for conservative management, thereby contributing to improved quality of life in pediatric patients.
