Título: ORAL MANIFESTATION OF LEPROSY: THE ROLE OF ORAL BIOPSY IN DIAGNOSIS

Resumo: A 65-year-old female presented with an ulcerated lesion on the hard palate, accompanied by multiple cutaneous nodules and a previous diagnosis of chronic granulomatous dermatitis without clinical improvement. Intraoral examination revealed a well-demarcated, erythematous, desquamative, ulcerated, and asymptomatic lesion measuring approximately 1 cm in the hard palate. Clinical differential diagnosis included squamous cell carcinoma, granulomatous conditions, and traumatic ulcer. An incisional biopsy of the oral lesion under local anesthesia was performed to establish a definitive diagnosis. Histological analysis showed a diffuse infiltrate of vacuolated foamy macrophages, suggestive of leprosy, and Wade staining demonstrated the presence of numerous acid-fast bacilli, confirming multibacillary leprosy. The patient was referred to the Infectious Diseases service, initiated multidrug therapy, and remains under clinical follow-up, with complete resolution of cutaneous and mucosal lesions and no complications. This case highlights the importance of oral biopsy in diagnosing systemic infectious diseases such as leprosy.
