Resumo: Spindle cell rhabdomyosarcoma is a rare variant of rhabdomyosarcoma that differentiates into skeletal muscle. 60-year-old man presented with an asymptomatic sessile nodular lesion on the dorsum of his tongue, measuring approximately 3 cm, with a suspected diagnosis of traumatic fibroma. The lesion showed slow growth and absence of associated pain or ulceration. An incisional biopsy was performed, and histopathological examination revealed a neoplasm composed of atypical pleomorphic spindle cells arranged in small fascicles. Immunohistochemical analysis demonstrated positivity for desmin and MyoD1, focal expression of myogenin, and negativity for CD34, S-100 protein, smooth muscle actin, and ALK. Together, these findings corroborated the diagnosis of spindle cell rhabdomyosarcoma. The patient was referred for oncological evaluation and multidisciplinary management to determine the most appropriate therapeutic approach. This unusual presentation in an older adult highlights the importance of clinicopathological correlation and immunohistochemistry in the differential diagnosis of spindle cell neoplasms of the oral cavity.
