Resumo: Leiomyoma is a benign smooth muscle neoplasm, usually occurring in the uterus or gastrointestinal tract, which rarely affects the oral cavity. A 39-year-old man was referred for evaluation with a 4-month history of an asymptomatic swelling in the tongue. His medical history was noncontributory. Intraoral examination revealed a normochromic, firm submucosal nodule on the left ventral surface of the tongue, measuring approximately 1 cm. Under diagnostic hypotheses of neurilemmoma and leiomyoma, an excisional biopsy was performed with high-power diode laser (980 nm, 2W, 605.85J) in continuous mode. Histopathologic analysis revealed a benign proliferation of ovoid and spindle-shaped cells arranged in interlacing bundles or haphazardly, interspersed with collagens fibers and blood vessels. Immunohistochemical analysis revealed positivity for alpha-smooth muscle actin (α-SMA) and low proliferation index (Ki-67 < 5%). The final diagnosis was leiomyoma. The postoperative course was uneventful. At 19 months of follow-up, there were no clinical signs of recurrence.
