Título: Keratocystoma of Minor Salivary Glands in the Palate: A Diagnostic Challenge

Resumo: A 23 year old indigenous female patient was referred to the Stomatology clinic at POUEA with a 7 year history of a painful palatal cyst. Intraoral examination revealed a nodular mass in the palate, exhibiting fibrous consistency and fluctuance. Aspiration biopsy was performed, and cytopathological analysis showed loose myxoid connective tissue infiltrated by basal cells with large, hyperchromatic nuclei, as well as myoepithelial cells.Incisional biopsy revealed orthokeratinized stratified squamous epithelium with underlying connective tissue containing multiple keratinized cysts lined by ductal epithelium, forming cords occasionally associated with myoepithelial cells and focal lymphocytic inflammatory infiltrate. The histopathological findings suggested a diagnosis of keratocystoma, with pleomorphic adenoma with squamous metaplasia considered as a differential diagnosis.Due to the unavailability of immunohistochemical evaluation for diagnostic confirmation, the patient was referred for excision of the lesion.
