Resumo: Oral hemangiomas are benign vascular neoplasms characterized by endothelial cell proliferation and variable clinical behavior, often necessitating intervention for functional impairment or aesthetic concerns. This case report details a 34-year-old female with an extensive reddish-purple hemangioma affecting the right buccal mucosa, lower lip and tongue congenital in origin but markedly enlarged post-pregnancy. Due to progression and esthetic compromise, high-powered blue-light diode laser photocoagulation was applied in right buccal mucosa using an 800 µm fiber at 2.5 W in continuous mode for 1.53 minutes under local anesthesia. The laser induces selective hemoglobin photocoagulation, leading to vascular obliteration with minimal damage to adjacent tissues. Postoperative follow-up revealed excellent healing, substantial lesion regression, enhanced oral function, and optimal cosmesis. This case highlights the effectiveness of diode laser as a minimally invasive, safe, and predictable treatment modality for oral vascular lesions, consistent with current literature.
