Resumo: Ameloblastic fibro-odontoma (AFO) is a rare mixed odontogenic lesion considered as an intermediate within the spectrum between ameloblastic fibroma and odontoma that affects posterior region of mandible in patients younger than 22yo. Here, we report a case of a 50yo man presenting clinically with an asymptomatic mass in the anterior region of the mandible causing displacement of adjacent teeth. Computed tomography revealed a unilocular hypodense lesion with ill-defined margins, expansion and destruction of the cortical bone. Under the hypothesis of glandular odontogenic cyst, an incisional biopsy was performed. Microscopically, large central areas of a myxoid and even hypercellular mesenchymal component resembling dental papilla admixed with cords and islands of odontogenic epithelium exhibiting peripheral columnar cells and deposition of dentin-like material were observed. Based on these findings, the final diagnosis was AFO. Treatment was the total removal of the lesion, and no signs of recurrence after 8 months of follow-up.
