Resumo: Carcinoma ex pleomorphic adenoma (CXPA) is a rare malignant neoplasm arising from the epithelial and/or myoepithelial components of a pre-existing benign pleomorphic adenoma, typically affecting patients in the sixth and seventh decades of life. We report the case of a 31-year-old male smoker presenting with an irregular nodule involving the hard and soft palate. The lesion had been present for 4 years, measured approximately 4 cm, exhibited a color similar to the adjacent mucosa and showed superficial telangiectasia. An initial diagnosis of pleomorphic adenoma had been rendered at another institution and an excisional biopsy was performed. Histopathological analysis revealed a biphasic salivary gland neoplasm composed of a proliferation of luminal and myoepithelial cells, with marked cellular pleomorphism and atypia among luminal cells and enlarged cells with irregular nuclei. The final diagnosis was CXPA and the patient was referred for oncologic management.
