Resumo: Melkersson-Rosenthal syndrome (MRS) is a rare neuro-mucocutaneous disorder characterized by the triad of orofacial edema, recurrent facial paralysis, and fissured tongue; however, complete presentation is uncommon. Moreover, histopathological findings are often nonspecific, with granulomatous inflammation is observed in a minority of cases, making diagnosis challenging. We reported a case of a 17-year-old male with persistent upper lip edema lasting over one year. Initially misdiagnosed as angioedema, he was treated with corticosteroids, but no allergen was identified. The condition progressed to swelling of the interdental papillae at teeth 11 and 21, prompting referral to an oral medicine specialist. Biopsy revealed nonspecific chronic mucositis. Considering clinical features and further investigation with a rheumatologist, Melkersson-Rosenthal syndrome was suspected and later confirmed through complementary exams. This case highlights the importance of clinical correlation, as incomplete presentations are frequent and may delay diagnosis and management.
