Resumo: Objective: To compare and rank the effectiveness of local and systemic interventions for xerostomia and salivary gland hypofunction in Sjögren's disease. Study design: A systematic search was conducted, followed by an ongoing network meta-analys (NMA). Outcomes included unstimulated (UWSF) and stimulated (SWSF) whole saliva flow, patient-reported oral dryness, Xerostomia Inventory (XI), Clinical Oral Dryness Score (CODS), and ESSPRI-dryness. Risk of bias was assessed using RoB 2.0. Results: A total of 86 randomized controlled trials (RCTs), 7,415 patients, and 22 interventions were included. Local interventions were evaluated in 35 studies and systemic therapies in 51. Based on available NMA results, conventional disease-modifying antirheumatic drugs (cDMARDs) showed the greatest improvement in UWSF (SMD = 3.57; 95% CI 1.58-5.56), followed by sialendoscopy with saline irrigation (SMD = 2.22; 95% CI 0.11-4.32). Stem cell therapy ranked highest for SWSF (SMD = 2.20; 95% CI 0.87-3.74). No intervention significantly improved xerostomia measured by XI. Risk of bias varied across studies, with more than half classified as low risk. Conclusion: Both local and systemic interventions improved salivary flow. However, effects were heterogeneous, and no intervention improved xerostomia. Standardized outcomes and high-quality trials are needed to establish comparative effectiveness and to inform clinical decision-making.
