Resumo: Adenoid cystic carcinoma (ACC) is a malignant salivary gland tumor that can affect the minor salivary glands, mostly the palate and buccal mucosa. Lower lip involvement is distinctly uncommon. Pain may represent an important clinical clue in ACC diagnosis, especially when the clinical features are nonspecific. We report the case of a 57-year-old male presenting with a progressively enlarging, painful submucosal nodule of the lower lip. Ultrasonography revealed an infiltrative pattern, and an incisional biopsy confirmed the diagnosis of ACC with cribriform architecture and perineural invasion. The patient was referred for oncologic treatment, undergoing surgical excision followed by adjuvant chemotherapy, and remains under medical follow-up. Although rare, ACC of the lower lip should be considered in the differential diagnosis of persistent and painful lip lesions. Early recognition and biopsy are essential to establish diagnosis and guide appropriate management.
