Título: Could Burning Mouth Syndrome Associated with Sjögren's Disease Be Considered an Overlap Syndrome?

Resumo: Overlap Syndrome involves the coexistence of conditions with clinical manifestations and mechanisms in one individual. Sjögren's Disease (SjD) is an autoimmune disorder affecting salivary and lacrimal glands, causing xerostomia and xerophthalmia, whereas Burning Mouth Syndrome (BMS) presents with oral burning without clinical alterations. These conditions share symptoms and pose therapeutic challenges, with low-level laser photobiomodulation therapy (PBMT) emerging as a non-pharmacological alternative for the management of both BMS and SjD-related manifestations. This report describes a 67-year-old woman who initially reported oral burning. Laboratory tests were normal, and PBMT was initiated for BMS after diagnosis. Subsequently, the patient reported xerostomia and reduced salivary flow, prompting further investigation. Unstimulated salivary flow rate was <0.1 mL/min. Minor salivary gland biopsy revealed chronic lymphocytic infiltrate with a focus score of 1.28, confirming SjD. Owing to overlap, PBMT was directed to burning sites and major salivary glands. Treatment improved salivary flow, reduced xerostomia, and burning.
