Resumo: A 13-year-old presented with swelling of the right maxilla; a dental abscess was initially suspected. On clinical examination, firm swelling of the vestibular sulcus was observed; it was painless on palpation and the mucosa was intact. A panoramic radiograph revealed tooth 18 displaced superiorly, projecting close to the lateral wall of the orbit. An incisional biopsy was performed, with the primary diagnostic hypothesis of odontogenic myxoma, which was confirmed by histopathological examination. Surgical planning included preoperative evaluation with laboratory tests, a cardiological evaluation, and computed tomography, which revealed an extensive expansive lesion associated with the impacted tooth, involving the maxilla, the orbital floor, the zygoma, and the ethmoid cells. Considering the patient's age and the extent of the lesion, a conservative approach was chosen, involving enucleation, curettage, superficial osteotomy, and reconstruction with mesh and titanium plates. The postoperative course showed satisfactory healing and an absence of symptoms.
