Título: RADICULAR CYSTS IN ADULTS AND OLDER ADULTS: A 20-YEAR RETROSPECTIVE STUDY FROM A SINGLE ORAL PATHOLOGY LABORATORY

Resumo: Objective: To investigate the epidemiological profile of radicular cysts submitted to biopsy, with emphasis on comparisons between adults and older adults. Study design: A retrospective observational study was conducted based on the analysis of 318 histopathological reports issued between 2004 and 2024 in an Oral and Maxillofacial Pathology laboratory. Results: A slight male predominance was observed (51.6%). The highest frequency occurred in the 50-60-year age group (23.6%), followed by 30-40 (21.1%) and 40-50 years (19.5%). Only 2.2% (n=7) were associated with deciduous teeth. In permanent dentition, upper incisors were most frequently involved (43.9%). Among cases with available data, symptomatic lesions predominated (52.6%), with frequent fistula presence (91.4%) and well-defined radiographic appearance (81.8%). Excisional biopsy without extraction was the most common procedure (39.9%), and the main clinical hypothesis was radicular cyst (66.7%). No significant differences (Fisher's exact test) were observed between adults and older adults regarding symptomatology, fistula presence, sex, lesion size, or previous endodontic treatment, although older adults tended to present shorter-duration lesions. Conclusion: Radicular cysts show a consistent clinicopathological profile across age groups, suggesting that aging per se does not substantially modify their clinical presentation. These findings support standardized diagnostic approaches regardless of age (Fapesp 2025/18801-7).
