Título: SCLEROSTIN EPITHELIAL IMMUNOEXPRESSION IN ORAL LEUKOPLAKIA AND ITS ASSOCIATION WITH CONNECTIVE TISSUE INFLAMMATION

Resumo: Objective: To evaluate epithelial immunoexpression of sclerostin in oral leukoplakia (OL) and its association with epithelial dysplasia (ED) and inflammatory infiltrate in the underlying connective tissue. Study design: A retrospective cross-sectional histopathological study was performed on 55 OL samples, including cases with hyperkeratosis (HKT) and with ED (low- and high-grade). Sclerostin immunoexpression was assessed using a semi-quantitative operator-based scoring system based on the percentage of positive cells and staining intensity (0-7), categorized as weak (0-3), moderate (4-5), or strong (6-7). Results: Sclerostin was predominantly expressed in the cytoplasm of keratinocytes, with occasional nuclear staining (mostly on HKT). Although stromal cells also showed positivity (inflammatory cells, endothelial cells, and muscle fibers), only epithelial expression was scored. Fisher's exact test revealed a significant association between epithelial sclerostin expression levels and the presence of inflammation (p=0.04), whereas no association was observed between OL subgroups and the presence of inflammation on the underlying connective tissue (p=0.34). Conclusion: Sclerostin shows diffuse epithelial expression in OL, predominantly cytoplasmic, and is associated with inflammatory infiltrate in the adjacent connective tissue.
