Resumo: Ameloblastic fibrodentinosarcoma is an extremely rare malignant odontogenic tumor characterized by a benign ameloblastic epithelial admixed with a sarcomatous stroma that may produce dentinoid matrix. A 12-year-old male presented with swelling and pain in the left mandibular region for 10 days, although the precise duration of the lesion remains unknown. Computed tomography showed a large expansile lesion involving the mandibular body and ramus, with cortical destruction, irregular margins, and internal hyperdense foci suggestive of calcifications. Incisional biopsy revealed islands and anastomosing cords of tall columnar cells with reverse nuclear polarity, embedded in a highly cellular stroma displaying marked pleomorphism, mitotic activity, dentinoid and myxoid matrix production, and dystrophic calcifications. Immunohistochemistry demonstrated a low Ki-67 proliferation index (<5%). The final diagnosis was ameloblastic fibrodentinosarcoma. The patient underwent surgical resection and is currently under follow-up. Due to its rarity and aggressive potential, early diagnosis and multidisciplinary management are crucial for optimal outcomes.
