Resumo: Basal cell adenoma (BCA) is a rare benign salivary neoplasm with biphasic
differentiation that may exhibit significant clinical and morphological similarities to
other benign and malignant salivary gland tumors. Here, we report a case of a 48yo man
who presented with a well-delimited, firm, painless and large swelling in the right
masseter region producing substantial facial asymmetry noticed approximately 16 years
earlier. Computed tomography showed a well-delimited round isodense homogeneous
soft tissue nodular mass with hypodense central area in the deep lobe of the parotid
gland. Under clinical hypothesis of pleomorphic adenoma and cystadenoma, surgical
intraoral excision was performed. Histopathological analysis revealed an encapsulated
benign neoplasm with a cystic appearance composed predominantly of basaloid cells
forming arranged in a mixture of solid, cribriform, and tubulotrabecular subtypes,
confirming BCA diagnosis. No neurological damage or local recurrence were noted
after 8 months of follow-up.
