Resumo: Male patient, 19 years old, 16 years post-renal transplant secondary to meningitis, on long-term immunosuppressive therapy, who was referred for evaluation of a suspected cystic lesion associated with the crown of a semi-erupted tooth 48. Panoramic radiography revealed a well-defined unilocular radiolucency associated with the distal aspect of the tooth. Computed tomography demonstrated a unilocular hypoattenuating lesion associated with disruption of the vestibular and lingual cortical plates, without root resorption. An incisional biopsy was performed in the gingival operculum overlying tooth 48, which exhibited bleeding and exophytic features. Histopathological analysis, along with immunohistochemical profiling (CD3, CD20, Ki-67, CD56, CD138, CD79A, CD163, kappa, lambda, MUM-1, ALK-1, and EBV), confirmed the diagnosis plasmablastic lymphoma. The patient was referred to a regional oncology center, where he underwent chemotherapy and radiotherapy. He has been disease-free since October 2025 and is under regular follow-up.
