Título: COCAINE-RELATED MIDLINE DESTRUCTIVE LESIONS: DIAGNOSTIC CHALLENGES AND MANAGEMENT OF TWO CLINICAL CASES

Resumo: Two clinical cases of palatal lesions associated with chronic cocaine use are
presented, both characterized by oroantral communication, infectious and
necrotic tissue changes, and significant functional impairment. Despite
differences in patient profile and therapeutic approach, both cases shared
similar clinical features, etiopathogenesis related to cocaine-induced ischemia
and tissue necrosis. Diagnosis was primarily based on clinical evaluation,
supported by patient history and imaging findings. Management included
antimicrobial therapy, prosthetic rehabilitation with a palatal obturator, and
antimicrobial photodynamic therapy (aPDT), highlighting the need for
individualized treatment strategies. Given the overlapping clinical presentation,
differential diagnosis should include granulomatosis with polyangiitis, malignant
neoplasms, and medication-related osteonecrosis. Cocaine-related midline
destructive lesions represent a diagnostic and therapeutic challenge due to their
chronicity, risk of progression, and significant functional and psychosocial
impact, requiring a multidisciplinary approach and long-term follow-up.
