Título: CLEAR CELL ODONTOGENIC CARCINOMA ARISING IN THE MAXILLA: CASE REPORT

Resumo: Clear cell odontogenic carcinoma (CCOC) is a rare malignant epithelial odontogenic neoplasm. Despite its often-indolent presentation, CCOC exhibits significant local aggressiveness. We report a case of a 51-year-old female presenting with a painless, slow-growing mass in the right anterior maxilla evolving over eight months. Intraoral examination revealed a firm swelling with focal erythema and telangiectasia of the overlying mucosa. Computed tomography demonstrated a 2.7 × 2.2 × 2.6 cm unilocular osteolytic lesion. Histopathological analysis of the incisional biopsy showed nests and cords of peripheral cuboidal eosinophilic cells and central clear cells within a prominent hyalinized stroma. By immunohistochemistry, the neoplastic cells expressed CK5/6, CK7, p40, p63, and p16, while remaining negative for S100. Ki-67 index was <5%. Following a diagnosis of CCOC, the patient was staged as T2N0M0 (stage II) and underwent a partial maxillectomy with 2.0-cm surgical margins. After a 1-year follow-up, the patient remains disease-free.
