Título: Castlemans disease associated with paraneoplastic pemphigus: challenges in the diagnostic process

Resumo: A 47-year-old male presented with hemorrhagic crusts on the lips and
widespread painful oral ulcers. Previous biopsies of an abdominal lesion and
buccal mucosa were inconclusive. Subsequently, a skin biopsy was performed,
suggesting pemphigus vulgaris. However, the severe clinical presentation and
the persistent abdominal mass raised suspicion of paraneoplastic pemphigus
associated with Castlemans disease. Following excision of the abdominal
lesion and a nine-month investigation, the final diagnosis was confirmed
through histopathology, immunohistochemistry, and immunofluorescence. The
patient was then referred for systemic treatment. This case underscores the
diagnostic complexity of rare conditions and the necessity of a multidisciplinary
approach. A thorough medical history, critical clinical observation, and precise
interpretation of complementary tests were essential to overcome initial
diagnostic hurdles and ensure appropriate management for these challenging
pathological entities.
