Resumo: A 70-year-old male was referred for evaluation of multiple white lesions on the oral mucosa with an estimated 10-year history. Clinical examination revealed asymptomatic, adherent white plaques with multifocal involvement of the bilateral buccal mucosa, vestibular fornices, and the anterior gingiva of both jaws, as well as the lingual gingiva of the anterior mandible extending to the left alveolar ridge. Incisional biopsies were performed in two representative areas. Histopathological analysis demonstrated hyperkeratosis associated with mild epithelial dysplasia in both sites. Based on clinical and histopathological findings, a diagnosis of multiple oral leukoplakias was established. Management consisted of staged vaporization using a high-power diode laser (infrared, 5W, short pulse). Photobiomodulation for analgesia and enhanced tissue repair was performed. The patient remains under follow-up, with satisfactory clinical outcomes and no evidence of recurrence in treated areas. High-power laser was essential for managing extensive multifocal oral leukoplakia, reducing the risk of malignant transformation.
