Resumo: Objective
To compare the relative frequency of micronuclei (RFMN) in intact oral mucosa and oral potentially malignant disorders (OPMD) in patients with Fanconi anemia (FA), and to evaluate its correlation with time post-hematopoietic stem cell transplantation (HSCT).
Study Design
Observational study including 46 FA patients divided into three groups: without HSCT (n=14), post-HSCT without OPMD (n=13), and post-HSCT with OPMD (n=19). Exfoliative cytology samples were collected from oral mucosa and OPMD lesions. RFMN was determined by manual counting. Non-parametric tests were used for intergroup and intra-individual comparisons, and correlations were assessed using Spearman and linear regression.
Results
RFMN in OPMD lesions was significantly higher than in the intact mucosa of the same patients (median: 8 MN/1000 cells; ~3.7-fold increase; p=0.001). No significant differences were observed in intact mucosa between patients with and without HSCT. A strong positive correlation was found between RFMN in intact mucosa and time post-HSCT (rho=0.694; p<0.001), independent of age and GVHD.
Conclusion
FA patients, particularly those post-HSCT with OPMD, exhibit increased genomic instability. The progressive increase in RFMN over time post-HSCT highlights the importance of continuous oral monitoring in this high-risk population.
