Título: PERIPHERAL OSSIFYING FIBROMA OF THE MANDIBULAR ALVEOLAR RIDGE: MANAGEMENT AND LONG-TERM FOLLOW-UP - A CASE REPORT

Resumo: A 54-year-old female presented with an asymptomatic gingival enlargement in the right mandibular alveolar ridge, noticed two months after multiple tooth extractions. Clinical examination revealed a firm, sessile, well-defined nodular lesion with mucosa-colored appearance and an ulcerated surface between teeth 43 and 44. The patient was partially edentulous and did not use a removable prosthesis. Periapical radiography showed radiopaque foci suggestive of mineralized content associated with adjacent teeth and residual roots, without evident bone destruction. Differential diagnoses included peripheral ossifying fibroma, ossifying fibroma, and pyogenic granuloma. Surgical management consisted of excisional biopsy combined with extraction of adjacent teeth and removal of residual roots, followed by local bone regularization and curettage. Histopathological analysis revealed fibrous connective tissue with central deposition of bone-like cementoid material, confirming peripheral ossifying fibroma. Healing was uneventful, with satisfactory tissue repair and normal mucosal appearance. Long-term follow-up up to one year and nine months demonstrated no recurrence.
