Resumo: Polymorphous low-grade adenocarcinoma is a malignancy of salivary glands predominantly affecting the palate of older female patients. This work presents a case report of an 82-year-old female patient referred for evaluation of a palatal lesion associated with a poorly maintained complete denture. The patient reported a hypertensive crisis and, upon seeking medical help, an extensive lesion was found. Intraoral examination revealed a soft, poorly defined swelling with superficial vascularization, located between the soft and hard palates. CBCT demonstrated a well-defined hypodense region behind the right maxilla. Clinical and imaging findings suggested benign and malignant salivary gland neoplasms. After incisional biopsy, histopathologic analysis confirmed a polymorphous low-grade adenocarcinoma. The patient was referred to a head and neck surgeon, and radiotherapy was initiated. After 10 months, local disease control was achieved. This report highlights the importance of early diagnosis of malignant lesions, linked to routine oral and general health care and self-examination.
