Resumo: A 37-year-old female patient was referred to an Oral Medicine service for evaluation of disseminated oral mucosal lesions present for approximately 90 days. She had previously been assessed elsewhere with a clinical suspicion of oral malignancy, despite negative rapid tests for syphilis, and was treated empirically with Nystatin, Dexamethasone, and Fluconazole without improvement. Extraoral examination revealed an ulcerated lesion in the left nostril. Intraorally, multiple painful ulcerated lesions were observed in the lower and anterior vestibule bilaterally, lateral and ventral tongue, and hard and soft palate, consistent with mucous patches. The tongue dorsum also showed rounded nodular lesions suggestive of condyloma lata. An incisional biopsy was performed, and histopathological analysis with positive immunohistochemical staining for anti-Treponema pallidum confirmed syphilis. The patient was referred for specialized care and underwent medical treatment with complete lesion regression.
