Resumo: Anti-tumor necrosis factor (TNF) agents, although widely used in the management of inflammatory diseases, may paradoxically induce granulomatous lesions that are clinically and histologically indistinguishable from sarcoidosis, known as drug-induced sarcoidosis-like reactions. This uncommon adverse effect contrasts with the therapeutic role of anti-TNF agents in refractory sarcoidosis. We report a 56-year-old woman with ankylosing spondylitis treated with golimumab who presented with painless whitish plaques and erythematous ulcerated areas on the upper attached gingiva, accompanied by fixed, painful submandibular lymphadenopathy. Panoramic radiography showed no bone involvement. Incisional biopsy was performed. Microscopical analysis revealed a non-caseating granulomatous inflammatory infiltrate, negative for special stains. Clinicopathologic correlation supported the diagnosis of a golimumab-induced gingival sarcoidosis-like reaction. Drug discontinuation led to clinical improvement, and the patient remains under multidisciplinary follow-up. Notably, such reactions may be restricted to the gingiva, posing a diagnostic challenge in clinical practice.
