Título: Diagnostic challenge due to malignancy mimetism in necrotizing sialometaplasia: report of case

Resumo: Necrotizing sialometaplasia (NSM) is a rare, benign, self-limited condition most commonly affecting the hard palate and often associated with a recent history of local trauma. It can mimic malignant neoplasms such as mucoepidermoid carcinoma (MEC) and squamous cell carcinoma, posing a diagnostic challenge. We report a case of a 46-year-old woman presenting with a three-week history of a hard palate ulcer. Based on the clinical hypotheses of NSM and MEC, an incisional biopsy was performed. Histological examination revealed an ulcerated area covered by a fibrinopurulent membrane, with small nests of squamous cells showing eosinophilic cytoplasm and prominent nucleoli. Ductal structures and mucous acini with PAS-positive cells were also observed, along with squamous metaplasia of salivary ducts. These findings supported the diagnosis of NSM. After 21 days, near-complete healing was observed. NSM may represent a diagnostic challenge for clinicians and pathologists, requiring careful clinicopathologic correlation.
