Resumo: Abstract
Medication-related osteonecrosis of the jaws (MRONJ) remains a challenging condition to manage, particularly in patients undergoing antiresorptive therapies. This study describes three clinical cases in elderly patients treated using therapeutic protocols based on photonic therapies. The patients had a history of bisphosphonate and/or denosumab use, presenting clinical manifestations characterized by bone exposure, suppuration, and the presence of sequestra, including one case associated with a dental implant. Management involved sequential approaches with antimicrobial photodynamic therapy (aPDT), photobiomodulation therapy (PBMT), and, when indicated, high-power laser (Er:YAG) for decontamination and localized osteotomy, with or without conservative surgical removal of the bone sequestrum. Complete clinical resolution was observed in all cases, with control of infectious signs and adequate tissue healing. These findings suggest that the integration of photonic therapies with minimally invasive interventions may represent a promising strategy in the management of MRONJ, contributing to reduced morbidity.
