Resumo: A 23 year old pregnant woman presented with a seven month history of progressive oral lesions associated with pain and anterior maxillary tooth mobility. Clinical examination revealed multiple erythematous, granular and friable nodular lesions involving the anterior maxillary gingiva, hard palate and upper lip, with extension to the nasal region. The multifocal infiltrative pattern raised clinical suspicion of paracoccidioidomycosis, leishmaniasis and lymphoma. Incisional biopsies from the lip and palate were performed. Histopathological analysis demonstrated pseudoepitheliomatous hyperplasia, granulomatous inflammatory infiltrate with multinucleated giant cells and numerous multiple budding yeast forms highlighted by PAS and Grocott stains, establishing the diagnosis of paracoccidioidomycosis. The patient was referred for systemic evaluation and antifungal therapy. This case highlights that oral paracoccidioidomycosis in pregnancy may present as extensive multifocal lesions mimicking infectious and lymphoproliferative diseases. Early biopsy and histopathological evaluation are essential for definitive diagnosis and appropriate management.
