Resumo: Florid cemento-osseous dysplasia may show complex behavior when associated with secondary lesions. A 45-year-old female patient presented with multifocal, predominantly radiolucent areas in the mandible. Longitudinal imaging over 13 years demonstrated dimensional changes, progressive internal calcifications, transition to a mixed pattern, and development of multiple bilateral intraosseous cavities. Cone-beam computed tomography revealed well-defined hypodense cavities associated with dysplastic areas. The teeth remained vital. Surgical exploration confirmed empty bone cavities without epithelial lining. The final diagnosis was florid cemento-osseous dysplasia associated with multiple simple bone cysts. Conservative management with clinical and imaging follow-up was adopted. During follow-up, episodes of pain and secondary infection were managed symptomatically, with maintenance of tooth vitality and stability of the lesions. Recognition of secondary cystic changes in florid cemento-osseous dysplasia is essential to avoid unnecessary interventions and to support periodic monitoring.
