Resumo:
An 82-year-old female presented with a lower lip lesion of unknown duration. Excisional biopsy revealed a glandular neoplasm composed of cystic spaces of varying sizes, lined by simple columnar epithelium with abundant mucinous cells arranged in papillary projections and luminal mucin accumulation. Mild focal cytologic atypia was observed, without mitotic figures. The stroma was scant and fibrous. Immunohistochemical analysis showed strong and widespread immunoreactivity for Ck7 and NKX3.1, a low proliferative index (Ki-67 <10%), and negative staining for myoepithelial markers (calponin, p63, and p40), supporting the absence of a basal/myoepithelial layer. These findings are consistent with intraductal papillary mucinous neoplasm (IPMN) of minor salivary glands, a recently recognized entity. This case highlights the diagnostic challenge due to its overlap with other mucinous salivary gland neoplasms and underscores the importance of morphologic and immunohistochemical correlation.
