Título: POLYMORPHOUS ADENOCARCINOMA OF THE SOFT PALATE WITH A SIX-YEAR CLINICAL COURSE: A CASE REPORT

Resumo: A 67-year-old melanodermic female patient, with no history of smoking or alcohol use, presented to an oncology referral center with a soft palate lesion initially diagnosed as squamous cell carcinoma (SCC) based on an external report. Intraoral examination revealed a sessile, firm, painless nodule, with color similar to the mucosa and purplish vascularization, measuring approximately 8 cm and evolving over 6 years. Due to clinical features atypical for SCC, a new incisional biopsy was performed, suggesting a salivary gland tumor. Histopathological analysis showed a malignant biphasic neoplasm infiltrating the lamina propria. Immunohistochemistry was positive for CK5/6, CK7, AE1/AE3, SOX10, and S100, raising suspicion of carcinoma ex pleomorphic adenoma or polymorphous adenocarcinoma. Induction chemotherapy with cisplatin and paclitaxel showed no significant response. The patient underwent partial pharyngectomy with extensive resection of the soft palate, adjacent structures, and left cervical lymphadenectomy. The final diagnosis, confirmed by immunohistochemistry, was polymorphic adenocarcinoma.
