Título: GRANULOMATOUS REACTION TO FILLER MATERIAL OF THE RIGHT BUCCAL MUCOSA: A CASE REPORT

Resumo: A 47-year-old woman presented with painless lesions in the right buccal mucosa. Additionally, she also reported a history of facial cosmetic procedures involving dermal filler materials. Clinical examination revealed two painless submucosal nodules, each approximately 0.5 cm, covered by normal-colored mucosa and located in the area corresponding to the previous aesthetic intervention. Excisional biopsy was performed. Histopathological analysis demonstrated an extensive granulomatous reaction, with macrophages and multinucleated giant cells surrounding rounded or polygonal eosinophilic structures of varying sizes. Contained crystalline material visible under polarized light microscopy, supporting the diagnosis of filler-induced granulomatous reaction. Postoperative follow-up at 7 and 14 days showed uneventful healing and the patient reported no symptoms or clinical complications. The case reinforces that filler reactions may mimic oral neoplasms, emphasizing the critical importance of a comprehensive anamnesis and meticulous clinicopathological correlation to achieve an accurate diagnosis.
