Título: ORAL HYGIENE WITH AND WITHOUT CHLORHEXIDINE: HAI-RELATED PATHOGENS IN VENTILATED ICU PATIENTS

Resumo: Objective: To compare the effect of toothbrushing with and without 0.12% chlorhexidine (CHX) on the frequency of pathogens related to healthcare-associated infection (HAI) in mechanically ventilated patients hospitalized in intensive care unit (ICU).
Study Design: A double-blind randomized pilot clinical trial was conducted with 70 orotracheally intubated patients, which were allocated into two groups: CHX (n=35, toothbrushing with 0.12% CHX) and Water (n=35, toothbrushing with filtered water). Both groups received standardized mechanical toothbrushing three times daily. Oropharyngeal swabs were collected at baseline (T0), after three days or until 24h-extubation (T1), and after seven days (T2) or until 24h-extubation. Microbiological identification was performed using MALDI-TOF. The frequency of HAI-related pathogens was compared between groups using Fishers exact test.
Results: No statistical differences between the groups were found in the frequency of individual HAI-related pathogen at any time point. The most frequently detected pathogens were Candida spp., Klebsiella spp., and Enterococcus spp. in both groups. Analyzing the groups individually, in the Water group, Klebsiella spp. frequency decreased significantly from T1 to T2 (p=0.041).
Conclusion: Oral hygiene protocol without CHX in the ICU intubated patients did not lead significant changes in oropharyngeal bacteria colonization by HAI-related pathogens.
